Chronic diseases, such as heart disease and diabetes, are the leading drivers of our nation's $4.1 trillion annual healthcare costs. Hospitalizations are a large part of the cost. Even before the COVID-19 pandemic, remote patient monitoring (RPM) was emerging as a promising way to improve outcomes for individuals with chronic diseases. This module will look at the benefits for both patients and clinical practice, which patients may benefit most, and ways for the healthcare team to overcome potential barriers in implementing remote patient monitoring within clinical practice.

Hypertension is known as the silent killer because it often does not show noticeable symptoms until it has caused significant damage. Regular and consistent screening is critical! Once identified, the healthcare team helps individuals manage their hypertension through weight and stress management, adhering to low-sodium diets, increasing physical activity, and taking prescribed medications. Current estimates show that only about 1/4 of people with hypertension have their hypertension controlled.

The activity purpose is to improve the knowledge and clinical skills of community primary care providers that treat children and adolescents with mental health. With both a national and Texas shortage of Child & Adolescent Psychiatrists, there is a great need for pediatricians to treat children with mental health. This series seeks to provide education to help those practitioners best identify mental health issues, develop care plans, and treat their patients, as well as how to identify when a referral to a psychiatric provider may be best for patient safety/care.
