HSC Health: 2022-24 Remote Patient Monitoring

Chronic diseases, such as heart disease and diabetes, are the leading drivers of our nation's $4.1 trillion annual healthcare costs. Hospitalizations are a large part of the cost. Even before the COVID-19 pandemic, remote patient monitoring (RPM) was emerging as a promising way to improve outcomes for individuals with chronic diseases. 

This module will look at the benefits for both patients and clinical practice, which patients may benefit most, and ways for the healthcare team to overcome potential barriers in implementing remote patient monitoring within clinical practice.

Target Audience

This activity is intended for physicians, nurses, pharmacists, certified public health professionals, social workers, physical therapists, licensed professional counselors, licensed marriage and family therapists and psychologists.

Learning Objectives

When you complete this module, you will be able to:

  • Describe the benefits of remote patient monitoring for the patient and healthcare team
  • Evaluate patient cases based on remote patient monitoring criteria to determine who would benefit most from remote patient monitoring. 
  • Identify ways various healthcare team members can support remote patient monitoring.
