Embracing the Journey: End of Life and Palliative Care
All seriously ill patients can benefit from palliative care principles in the primary care setting. This session is designed to offer clinicians the information and skills needed to provide high-quality primary palliative care to patients with serious illness. Participants will develop a toolbox of palliative care competencies including pain and symptom management and serious illness communication skills.
Learning Objectives
- Differentiate between patients who would benefit from palliative care and those who would benefit from hospice
- Outline common serious illness trajectories
- Describe a multidimensional approach to assessing and managing pain and other symptoms in serious illness
- Develop strategies for discussing and documenting goals of care in the primary care setting
